Contemporary reproductive health landscape: A review and proposal for a conceptual framework

Abstract:

Reproductive health is a critical aspect of public health, influenced by global and regional epidemiological trends, social determinants, economic disparities, and cultural norms. Despite significant progress, emerging challenges such as adolescent sexual health risks, infertility, maternal mortality, and the burden of sexually transmitted infections persist. These issues are further compounded by gender norms, educational inequities, and limited healthcare access, which disproportionately affect women, adolescents, and marginalized populations. Economic barriers exacerbate reproductive health inequities, restricting access to essential services and quality care.
Policy and programmatic responses at global and national levels have attempted to address these challenges through initiatives aimed at improving access to family planning, safe motherhood programs, and adolescent reproductive health services. However, inconsistencies in policy implementation, gaps in service delivery, and sociocultural resistance hinder progress. Addressing these gaps requires a more holistic and youth-friendly approach that integrates sociocultural adaptability, gender equity, and comprehensive education.
This review proposes the Olofinbiyi Reproductive Health Conceptual Framework (ORHCF), which employs the "House Metaphor" to enhance understanding of reproductive health especially among adolescents and young people. By structuring reproductive health into foundational, structural, and protective elements, this framework provides a novel, visual, and contextually relevant model for guiding interventions, policy development, and research. The framework offers comparative advantages over existing models by emphasizing the interconnected nature of reproductive health determinants, making it an adaptable tool for shaping future reproductive health policies and programs.
